​When you book your appointment, we will text or email you an intake form link to be completed virtually. If you are unable to do this for whatever reason, please come to your appointment 20 minutes early.​
If you are attempting to book a new patient exam and treatment and you are using Worker's Compensation, Personal Injury, or Auto claim, please call the office to schedule your appointment.

NO-SHOW / LATE CANCELLATION FEE
At Makos Chiropractic we understand that life happens and sometimes it is difficult to keep an appointment. However, consistent no-shows and last-minute changes do hurt business as we are unable to fill those time slots with such short notice. If you must cancel or reschedule your appointment, please do so 24 hours or more in advance so we have time to contact patients who are waiting for a session. We reserve the right to charge a $20.00 fee for any appointment that is canceled or rescheduled less than 24 hours ahead of time. Thank you.
