Due to having a waiting list to get an emergency appointment and an increasing number of no-shows, we now require a credit card to be held when booking as a new patient for the first time. Existing patients can schedule as usual in the new system.
Our system, Square is very secure. If you show up to your scheduled appointment you have nothing to worry about. We have no access to your credit card information. As stated on the home page, "we reserve the right to charge a $20.00 fee for any appointment that is canceled or rescheduled less than 24 hours ahead of time".
If you do not have a credit card, please call the office to schedule. We appreciate your understanding!
Please print off our intake form to fill out prior to your appointment to save time once you arrive! If you are unable to print and complete the paperwork, please come to your appoint 15 minutes early.
If you are attempting to book a new patient exam and treatment and you are using Worker's Compensation, Personal Injury, or Auto claim, please call the office rather than continue here. There is more information we will be needing from you before scheduling.
