​When you book your appointment, we will text or email you an intake form link to be completed virtually. If you are unable to do this for whatever reason, please come to your appointment 20 minutes early.​

 

If you are attempting to book a new patient exam and treatment and you are using Worker's Compensation, Personal Injury, or Auto claim, please call the office to schedule your appointment.

We are no longer in network with GHP family (medical assistance). We are still in network with all other Geisinger plans.
